You may obtain a certified copy of a Death Certificate by providing the following:

  1. Your full name with proper spacing.
  2. Exact name of the person for whom you are seeking the record (or exact name of a child on record).
  3. Date of death
  4. Place of death
  5. Father´s Name
  6. Mother´s Name
  7. A photocopy of your Photo I.D. (i.e. Drivers License or Military I.D.)

The cost is $23.00 for the 1st copy and $8.00 for additional copies of the same record. Your check or money order should be made payable to "Madison County Clerk".

ILCS410/535/25/4(d) states that copies of death or fetal death records may be issued upon:

• The specific written request for an uncertified or certified copy by a person, or his duly authorized agent, having a genealogical, (record must be over 20 years old), personal or property right interest in the record.

Individuals must be 18 years or older in order to obtain a death record.

DELIVERY FEES: Delivery is provided free by USPS or you may upgrade to FEDEX OVERNIGHT for an additional $40.00.  Delivery payable by major credit card only American Express®, Discover®, MasterCard® or Visa®.

Death records cannot be faxed
